It is worth doing when a problem recurs, when a stop is expensive, or when the obvious fix has already failed twice. It is overkill for genuine one-offs.

The core methods

  • 5 Whys. Ask "why" repeatedly until you reach a cause you can change. The discipline is to keep going past the first satisfying answer, and to follow evidence rather than opinion.
  • Fishbone (Ishikawa). A diagram that sorts possible causes into categories (machine, method, material, people, measurement, environment) so the team explores the whole space instead of fixating on one branch.

Immediate trigger versus root cause

Every failure has layers. The immediate trigger is what stopped the line this second (a jam). The root cause is why that trigger keeps occurring (a misaligned guide installed during a changeover). Fixing the trigger restarts the line; fixing the root cause stops the recurrence. Teams that confuse the two stay busy without getting better.

Where RCA goes wrong

  • Stopping too early. The first plausible cause is rarely the root. "Operator error" is usually a symptom of a process that allows the error.
  • Starting from bad data. If downtime reasons were guessed under pressure, the Pareto points at the wrong cause and the whole analysis is wasted. Accurate capture comes first; see downtime versus uptime.
  • No follow-through. A root cause identified but never turned into a tracked fix changes nothing.

Making findings stick

The output of an RCA should be a specific, owned, tracked action, not a memo. In practice that means the conclusion becomes a work order or a change to the preventive maintenance schedule, with a date and an owner, and the recurring fault is monitored to confirm it actually stopped.
