Who should be involved in a 5 Whys analysis?

You need a small, cross-functional team. This must include the people who are closest to the problem, such as the operator who runs the machine and the maintenance technician who fixes it.

How long should a 5 Whys meeting take?

For a well-defined problem, the meeting should be fast and focused, ideally 15 to 30 minutes. Having instant access to OEE and CMMS data is the key to getting a fast, accurate answer.

Step 5: Prove the Cure Worked

An RCA is finished when the fault stops, not when the task is closed. Put a date four to six weeks out and check the downtime Pareto for Press #7: has the motor overload dropped off the list? Check the asset history for new motor alarms and confirm the lubrication PM has been done on time since the change. If the fault is back, the chain of whys stopped too early or the fix was not the real one, so reopen the analysis with the new data.
